Adult Release: This release form is for riders that are 18 years of age or older. Please read form and fill out in full. Each form contains room for five riders. This form must be brought to sign in with proof of ID and your Crow Canyon card. Crow Canyon cards may be obtained at sign in.

Minor Release: This release is for minors under 18 years of age. If you are the parent or legal guardian please read and fill out this form in full. One form per minor. Note: This form does not need to be notarized if parents or guardian is present at sign in.

 

If you are bringing a minor child that you are not the parent or legal guardian of, the parents or legal guardian of the minor child must read this form and fill it out in full. This form must be notarized and proof of ID must be shown at sign in. NO EXCEPTION! If this form is not completed in full the minor child will not be permitted to ride.
